Position Summary:
Reporting to the Restorative Care Coordinator, the Restorative Care Assistant is responsible for the implementation of plans of care that focus on optimizing resident independence in activities of daily living, as well as assisting in the transfer of knowledge of restorative care plans to the nursing rehabilitation team.

Qualifications:
• Completion of a recognized post-secondary degree and/or two year diploma with a restorative focus at the community college level in a health related field inclusive of Kinesiology and/or PTA/OTA assistant program preferred
• Related Health care diploma with relevant restorative course supplemented by the restorative care certificate program minimum
• Good oral and written communication skills;
• Ability to follow direction and work in a team environment;
• Knowledge of the specific needs of older adults diagnosed with dementia;

• Complete assessments of residents; establish resident goals and interventions based on assessments and needs.
• Complete Documentation related to Restorative Care, including but not limited to assessment, daily flow sheets, progress notes, input into care plans, evaluations and discharge.
• Participate in the homes Continuous Quality Improvement program including but not limited to completing audits and ongoing program evaluation.
• Ensure effective communication with all residents, family members, staff, and volunteers.
• Work collaboratively as part of a multidisciplinary team, including but not limited to the resident, family, nursing, dietary, environmental, activation, PT, OT, and vendors to ensure interventions to promote resident independence are implemented.
• Assist in processes related to Physiotherapy, Occupational Therapy and therapy supply and service providers including but not limited to seating, assistive devices and mobility aids.
• Participate in Restorative Care Committee and Multidisciplinary Care Conferences when required.
• Provide educational opportunities related to Restorative Care as needed.
• Comply with all company policies, procedures and practices.
• Maintain awareness of changes within the profession and seek continuing educational opportunities.
• Other duties as assigned

To be eligible for this assignment, you must have a degree in Kinesiology and experience in Restorative Care OR Completion of a Physiotherapy or Occupational Therapy Assistant Program OR Degree/Diploma in Recreation/Activation and Certificate in Restorative Care from a recognized educational institution OR PSW Certificate and Certificate in Restorative Care from a recognized educational institution Or combination of the above. You must have experience working in a Restorative Care Setting with program planning, documentation and evaluation. You demonstrate, initiative, the ability to work independently and or in a team setting.
